Jacqueline "Jackie" O'Connor, 86 of Cookeville, Tennessee passed away on Tuesday, August 2, 2022, at her home.

Jackie was born Sunday, December 29, 1935, in Berwyn, Illinois, and she was the daughter of the late John Zacek and the late Lillian (Klotz) Zacek.

Jackie was a loving sister, wife, aunt, and friend.

Jackie is survived by, 2 Nieces, Debra and Peter Czech and Christine and Kris Smith; 2 Great-Nieces, Christina and Damon Olson and Patricia and Vytas Vaznelis; Great-Nephew, Sam Smith; 2 Great-Great-Nieces, Josephine and Jacqueline Olson; Great-Great-Nephew, Andrew Olson; Special Caregivers, Janice and Mike Nye, Mikalah Crumby, and Daisy Wagner.

In addition to her parents, Jackie was preceded in death by, Husband, Matthew O'Connor; Sister, Rose Bernatsky.

Memorial Gathering for Jacqueline "Jackie" O'Connor will be held Wednesday, August 10, 2022, from 5:00 pm until 8:00 pm at Crest Lawn Funeral Home.
